1. The Generic Personal Statement (GPS) can be used when applying for the following covers under ING’s OneCare:
   - Life Cover
   - Total and Permanent Disability Cover (Any, Own and Home-maker definitions only)
   - Trauma Cover
   - Income Secure Cover
   - Business Expense Cover (Section C5 of the OneCare Application Form must also be completed)
   - Child Cover (Section A1 ‘Child to be insured’ and Section C12 of the OneCare Application Form must also be completed).
   
   
   If the GPS is completed, the life insured does not need to complete the Life Insured’s Personal Statement (pages 6-17 of the OneCare Application Form unless they are applying for Business Expense Cover or Child Cover (see above).
   
   
2. The GPS should be attached to ING’s current OneCare Application Form with pages 1 to 5 and 18 to 21 completed where appropriate.
   
   
3. The GPS is not suitable if applying for:
   - Total and Permanent Disability Cover (Non-working definition)
   - Living Expense Cover
   The Life Insured’s Personal Statement (found in the OneCare Application Form) must be completed when applying for these covers.
