  1. Please complete Sections 1 to 3, Personal Statement - Telephone Underwriting and Sections 26 to 34 of the Tower application.

  2. When completing Question 66 Income Details, of the GPS, refer to Tower specific 'add-backs', Section 9 of Tower application.

  3. BUSINESS EXPENSES INSURANCE - please complete Section 10 of Tower application.

  4. If MEDICAL EXAMINATION required, Tower require all medical questions on the GPS to be completed. This is standard Tower requirement irrespective of whether GPS is used or not.
