GENERIC PERSONAL STATEMENT (GPS)

- If all risk business for a client is being placed with Prefsure, they request that their application be used rather than the GPS.
- If parts of the GPS are not completed because a medical examination is required, the applicant must sign a statement to the effect that the medical examination will form part of the application to Prefsure.
