  1.  AXA have indicated they will only accept the GPS when concurrent business is being written with another insurer.

  2. Ensure product and other sections of AXA application are completed, including pages 3 - 9 and pages 25 - 35 as appropriate.

  3. Signatures are required on page 29 for Non-Superannuation applications or pages 30 to 33 for Superannuation applications.
