Additional Instructions

1. AXA have indicated they will only accept the GPS when concurrent business is being written with another insurer.
   
   
2. Ensure product and other sections of AXA application are completed, including pages 3 - 4 and pages 25 - 26 as appropriate.
   
   
3. Signatures are required on page 29 for Non-Superannuation applications or pages 33 to 35 for Superannuation applications.
