In all cases:

• Fully completed - AIA Quote
• Section A (page 1 of 24) - AIA App.
• Section X (page 21 of 24) -  AIA App.
• Adviser use only (page 22 of 24) - AIA app.
• Direct Debit Request, Credit Card authority, Authority to release Medical Information (page 23 of 24) - AIA app.
  

As applicable in AIA Application:

• Section B (policy owner (s) - Policy 1 (Non Superannuation) (page 2 of 24)
• Section C Policy details - Policy 1 (Non Superannuation) (page 3 of 24)  
• Section D,E,F (page 3 of 24) 
• Additional Question for any type of AIA Australia’s Crisis benefit or Crisis Stand Alone (Females only). Click here for Additional Question form. 
• Section N (Family Protection) to be completed if purchasing this benefit (page 11 & 12 of 24) 
• Section V (page 19 of 24) 
• Section W (page 20 of 24)

All of the above sections are required to be completed from the AIA application form together with a completed GPS application.

Important: When application is submitted to AIA all of the above AIA criteria must be stapled in front of the completed GPS application.
